D.1 Scope and pricing. The records retrieval services FCA offers, their scope, turnaround options and prices are on the product page at https://fundcapitalamerica.com/services/law-firm-services/medical-record-retrieval/, as posted when the request is submitted. A signed pricing agreement controls over the page.

D.2 Authorization and HIPAA. The customer must supply a valid signed authorization or other legal basis for each request. FCA and its providers handle records with appropriate safeguards.

D.3 Provider response. Medical providers decide whether, when and at what cost to produce records. FCA does not control provider response times, completeness or fees, and may pass provider charges through to the customer. Records are provided as received.

D.4 Records; retention. Records are covered by Master Terms Section 5.5. Records and request files are kept for 24 months after completion, then deleted or de-identified, subject to legal needs.
